I just noticed in my Planescape stuff that there is a copy of a map with Tcian Sumere, The Vault of the Drow, The Astral Plane, and Mysteries of the Stones on an 8 page map.
Is this from Dead Gods? If so, please tell me ASAP!

Is it a fold out map or a booklet. Everything I've read says that the map is a large fold-out.

It's a fold out.
I posted the book today, then noticed the fold-out map just now. Obviously the map must have fallen out when I picked the book up to wrap!

AFAIK, Dead Gods does not have any maps.

According to the TSR Archive, Dead Gods includes a fold-out map.

"Many point out that this battle against the darkness is an impossible one, and they may well be right. Impossible or not, however, the battle for righteousness is one that only a few heroes are brave enough to undertake"

My copy of Dead Gods has a fold out map (perforated) in the back.  Mine is still  attached so I'll tell what I can see without removing it: Crux, Vault of the Drow, Tcian Sumere, The Battle Field and The Temple.  It has 8 pages that are a bit smaller than the book itself and is single sided.

Thanks for the replies.
Someone had bought the book from me (which I posted yesterday afternoon), and I didn't notice that when I picked it up to wrap, that the map stayed on the shelf.
It's a good job that I needed to check my other Planescape stuff later in the day and noticed the map! I posted it today. So all's good.

Just in case you wanted to be sure, this is the fold-out map from Dead Gods fresh off the digicam:

Hope it helps. :)

That's what I was asking about, Azurah! That's what I had.
