Although they did some extensive playtesting of the 3.0 products, it was inevitible that some loopholes were to be found.
i.e. The 3.0 version of the 4th level spell Polymorph Self allowed you to poly into a Solar with their natural AC bonus and other goodies (no spellcasting abilities though, no vorpal blade, etc.), or polymorph into some other creature with a high armor class. The 3.5 version now is retricted by the casters hit dice.
The 3.0 version of the Haste spell allowed casters to cast two spells per round. Add Quickened Spell to that and casters were chucking 3 spells per round. The nerf bat swung heavy and low into this one (and rightly so).
With the change from 3.0 to 3.5, all the splat books needed to be changed as well. Sword and Fist became The Complete Warrior, Tome of Blood became The Complete Arcane.
