Ok, this one has got me!!  Maybe you guys can help me with it.  I'll go through it by the numbers and let me know what you guys think.  Wizard logo, "Advanced D&D" in angled yellow banner in upper left corner of cover (D is downsized and not running off edge), Wizard logo and TSR address on the spine, spine inlay is no longer red/yellow stripped, endpapers and flyleaves are white, 232 pages and page 99 number might be closer to the bottom of the page.  "Printed in the U.S.A" on copyright page.

Sounds like a possible print run to fix for the wrongly inserted MM pages to me.  Got some pics? :D
