I found this at my mom's place buried with a bunch of old character sheets.

Anybody know if this came in a boxed set... and which one?

Looks folded in thirds, so, probably came in an envelope, maybe with a Dragon reminder to resubscribe?

Yes it's folded like a letter.

I never had any Dragon mags... For some reason my mind keeps thinking Expert set or Fiend Folio... But the envelope rings a bell.

GENCON XIII was in 1980.

Perhaps it came with this pre-registration booklet?
