There is no printing rank stated on the inside cover. Just the copyright, 1974 Tactical Studies Rules. Cover and pages are super clean (IIDKB, I would say its a reprint), clearly NM rating. But two things strike me as odd:

No page numbers for 5, 9, 11, 14, 16, 19, 20, 24, 36.

The illustration on (what would be) p.14, shows a 4 member party: Fighting Man, Magic user, Thief, Cleric....thief?!!...but, but that doesn't show up as a class until Feb. 1975 and the Greyhawk Supplement I...

I'm confused.

Your description exactly matched my OCE M&T.  All the page numbers omitted match and so does the illustration.  The OCE I have was the 7th print based on Acaeum guidelines.

Thork, the illustration on page 14 of the four member party is by Tom Wham, and replaced the monster entry for "Balrog".
The picture of the Balrog is on the previous page and was left in, but the name "Balrog" was removed from the illustration.
IIRC, this was done when the other Tolkien references were changed (late 1977?). More info on the Balrog illustration here:
