The shrinkwrap is not original. These modules were released in two boxes with a 'big book of old' appearance.

Hamilcar has this one up as well and most of the other TSR boardgames:

I looked over a copy at Value Village one time. Its a kids game in which you need to make potions. The cards themselves had fairly nice pictures of different coloured potions though.
